Gabapentin was born as a seizure medication, moonlights as a nerve-pain drug, and somewhere along the way started showing up in anxiety treatment plans. If a prescriber has mentioned it, or a friend swears by it, and you're wondering why an epilepsy drug is in this conversation at all, that's the right question to ask.
Why prescribers reach for it off-label
Gabapentin calms overactive nerve signaling, and clinicians noticed decades ago that this settling effect extends to some anxious brains. It is not FDA-approved for anxiety, so this is off-label prescribing, which is legal, common, and requires honest framing: the evidence base is thinner than for first-line treatments, strongest for social anxiety and as an add-on when SSRIs get partway there. It is also not a controlled substance federally, doesn't carry benzodiazepine-style dependence risk at the same level, and doesn't require the escalating doses that make benzos a long-term trap. Where it genuinely helps
